Complete Application for New Members
We ask all of our new patients to complete a new member application as completely as possible. This form includes questions about your health history and gives us an understanding of past trauma and injuries that might be contributing to current health problems.
A comprehensive chiropractic evaluation and Neurological Diagnostic Testing will be performed to further determine the exact cause of your health problems. Using our state-of-the-art diagnostic equipment to locate the root cause of any dysfunction or disease within your body. Evaluating your nervous system and muscles allows us to discover where pressure and spinal misalignments might be, and how we can best correct them.
At this point we will take any postural x-rays, or other diagnostic imaging that may be required, so we can more closely examine your skeletal system. This will confirm preliminary diagnoses and allow us to approach adjustments and care with the most detailed information possible.
Principled Doctor’s Report and Report of Findings
This is your most important appointment! On your follow-up visit, we will sit down with you and present a detailed report of our findings. This will include analysis of your diagnostic imaging and the results of your examination.
After reviewing your case and the cause of your health issues, the doctor will decide if you are a candidate for chiropractic care and a thorough explanation of care recommendations and action steps will be explained.
